Internal Reporting Tools
Provided by Waubonsee Community College
The college provides reporting tools for students, faculty and staff to make an official report of a situation or incident that took place on or off campus involving Waubonsee students.
Waubonsee Intervention Form (WIF)
The Waubonsee Intervention Form is used to report general incidents, such as behavioral issue, threatening behavior or a violation of the Code of Student Conduct.
Harassment, Discrimination or Retaliation Form
This form is for use by individuals to file a formal complaint for Harassment, Discrimination or Retaliation.
Sex Discrimination, Sex-Based Harassment or Retaliation Form
This form should be completed to report Sex-Based discrimination, Sex-Based Harassment or Sexual Misconduct or Retaliation. For additional information visit the Title IX webpage.
Student Complaint Form
The Student Complaint Form should be completed when a student, parent or community member has a written concern or charge of dissatisfaction with a person, service or process within Waubonsee Community College.
Academic Misconduct Form (For Faculty)
The Academic Misconduct Form is used to report incidents of cheating, plagiarism or other academic misconduct.
Whistleblower Reporting
Visit the Whistleblower Reporting site for more information or to report allegations of misconduct on the part of the board of trustees, employees, representatives speaking or acting on behalf of the college, and volunteers.
External Reporting Tools
External reporting tools are also available to students, faculty, staff and community members to submit a complaint about an institution.
Submit a complaint to Higher Learning Commission (HLC) about an institution
Students, faculty, staff and members of the public may submit a complaint about an HLC-accredited or candidate institution.
Filing complaints about an Illinois Community College with the ICCB
If you attempted to file a complaint at the institutional level and were not successful, you may file a complaint with the Illinois Community College Board.
